Croatia must defeat Ukraine in the upcoming game, and that will probably be enough only for the second place in the group since Iceland will play against outsiders Kosovo at home in their final game. If they record a draw, Croatia will stay in front of Ukraine, but with 18 points there is a high possibility that they would be the worst second placed team, and therefore miss the additional qualifications.
